Introduction

INTERVENTIONS TO PREVENT HIV RISK BEHAVIORS

Our nation is facing the most critical and devastating epidemic in recent history. In 1992 it is calculated that nearly two million years of potential human life were lost as a result of premature death from AIDS, which surpasses loss of life attributable to stroke, heart disease, and cancer. HIV infection and its clinical complications are placing a heavy strain on medical and social services in both the public and private sectors. One in 250 people in the United States is infected with HIV, and AIDS is the leading cause of death among men and women between the ages of 25 to 44. In the U.S. gay men, or men who have sex with men, still account for the largest number of cases, but the rate at which women are becoming infected is higher. The percentage of AIDS due to heterosexual contact increased by 21 percent from 1990 to 1991 and continues to escalate. HIV infection in nearly one third of seropositive people is related to injection drug use.

Beginning in 1983 the public health system has supported research to develop behavioral approaches to prevent the further spread of the virus through changes in high risk sexual behavior and injection drug use behavior. Significant progress has been made in understanding how to help individuals change their AIDS-related risk behaviors. Based on a variety of models of behavior change, including social learning theory and related health and substance abuse models, these behaviorally-based interventions begin with AIDS and substance education as necessary components, but also include skill acquisition, assertiveness training, and behavioral reinforcement components. In recent years research documents that aggressive implementation of HIV reduction efforts to promote safer sexual behavior and prevent substance abuse could avert tens of thousands of new HIV infections and potentially save millions of dollars in health care costs.

This bibliography was prepared in support of a National Institutes of Health Consensus Development Conference on Interventions to Prevent HIV Risk Behaviors held in Bethesda, Maryland on February 11-13, 1997. Behavioral and social scientists, prevention researchers, statisticians, and methodologists as well as other health care professionals were brought together to review the data and evaluate the interventions to prevent the further spread of HIV/AIDS. The Consensus Development Conference was timely because behavioral interventions are currently (and for the foreseeable future) the only effective way of containing the further spread of HIV infection. It is critical to identify effective risk reduction behavior change interventions, understand their effectiveness in different settings with different populations, and ensure that the best tested interventions are being used in the public health system to help people make and sustain behavioral changes to reduce their risk for contracting HIV infection. Public health personnel need to be delivering the most effective behavior change interventions or they can become discouraged if clients continue to seroconvert and become ill despite their efforts.

Although the items selected are primarily in English, all languages were searched.
